San Bernardino, California (April 15, 2021) – The San Bernardino Sheriff-Coroner’s Bureau has identified the man who died Wednesday afternoon following a two-vehicle crash in San Bernardino as Juan Ochoa, 50, of Fontana.

The deadly accident was reported at around 2:55 p.m. Wednesday, April 14, at the intersection of West 5th Street and North G Street.

Two vehicles were reportedly involved in the collision.

One of the parties involved, now identified as Ochoa, was taken by emergency responders to Loma Linda University Medical Center, where he later succumbed to his injuries at 3:56 p.m.

No additional information about the accident was provided.

The San Bernardino Police Department is investigating the crash.
