Fabian Castellanos Killed in DUI Head-On Crash on Road 92 in Tulare
Fabian Castellanos ID’d as 19-Year-Old Visalia Man Killed in Tulare Collision on Road 92 and Avenue 260
Tulare, California (July 26, 2021) – The teenager who died following a head-on crash involving a drunken driver Saturday night near Tulare has been identified as Fabian Castellanos, 19, of Visalia.
The deadly accident occurred at about 10:45 p.m. Saturday, July 24, on Road 92 and Avenue 260, according to the California Highway Patrol.
The CHP said Jerry Diaz Avila, 21, of Tulare, was driving northbound on Road 92 when he failed to navigate the turn near Road 92 and Avenue 260.
Avila’s vehicle reportedly drifted into the southbound lane, where it collided head-on with a pickup truck driven by Castellanos.

Castellanos was ejected from his vehicle upon the impact. He was pronounced dead at the scene.
A 19-year-old woman, who was riding as a passenger in Avila’s car, sustained major injuries in the crash. Her condition at this time remained unknown.
Investigators determined that Avila was under the influence of alcohol at the time of the accident. He was arrested and booked into jail for suspicion of DUI and manslaughter.
The crash remains under investigation.
