Woman, Boy Killed in Multi-Vehicle Crash on 101 Freeway in Sherman Oaks
40-Year-Old Woman and 15-Year-Old Boy Killed in Sherman Oaks Crash on 101 Freeway near Van Nuys Boulevard Exit

Sherman Oaks, California (April 25, 2021) – Authorities are investigating a Saturday morning multi-vehicle accident that killed a 40-year-old woman, 15-year-old boy and left several other parties injured in Sherman Oaks, authorities said.
The accident occurred at around 12:10 a.m., on April 24, on eastbound 101 Freeway, near the Van Nuys Boulevard exit.
According to the California Highway Patrol, a 2008 Honda, driven by a 20-year-old woman, was headed eastbound on Van Nuys Boulevard when it rear-ended a 2019 Isuzu box truck.
Following the collision, both vehicles drifted to the center divider wall. The Isuzu entered the westbound lanes and was struck by a 2017 Honda. A 2016 Dodge, also in the westbound lanes, was struck by concrete debris caused by the destruction of the center divider wall.
The 40-year-old woman was pronounced dead at the scene. The 15-year-old boy was taken to Ronald Reagan UCLA Medical Center, where he later died from his injuries. They were both passengers in the Isuzu.
The driver of the 2017 Honda, a 49-year-old woman from Reseda, was rushed to Ronald Reagan UCLA Medical Center with major injuries.
The wreck prompted a SigAlert closing the Nos. 1, 2, and 3 lanes in both directions for an unknown duration.
Authorities are investigating to establish the cause of the crash.
